 follicular or simple cyst는 3cm 까지

 corpus luteum cyst도 정상소견임

 simple cyst가 암이 되는 경우는 사실 없다. 하지만 크기가 7.5cm넘으면 mural nodule을 찾는데 어려움이 있다. 그러므로 MRI나 수술을 해야 한다.

 post menopause에서는 1cm 미만은 정상, 1~7cm까지는  일년마다 FU해야 함.

 paraavarian and paratubal cyst도 비슷하게

application of DECT

인체 조직은 80Kvp와 140Kvp에서 attenuation이 linear한데 반하여 iodine은 80 KvP에서 attenuation이 급격히 증가한다. 이러한 성질을 이용해서 CT를 dual energy로 찍으면 iodine map을 얻을 수 있다.
1. iodine  map을 이용하여 renal cyst가 실제로 조영증강되는지 아니면 high density cyst인지를 알아낼때
2. lung의 perfusion map
3. angiography에서 뼈를 발라낼때
4. renal stone중에서 urine alkalization해서 녹일 수 있는  uric acid stone ( 약 15%, calcium이나 struvite stone과는 달리 photoelectric effect가 없다)을 골라낼때

쓸수 있겠다.

 80kvp+140kvp fused image 에서 iodine distribution image를 subtraction하면 virtual pre-contrast image를 얻을 수 있다.

uric acid stone은  광전효과를 일으킬 만한 성분을 포함하고 있지 않아서 구별이 가능하다.

Acute pancreatitis

acute pancreatitis의 분류: interstitial edematous pancreatitis는 췌장이 약간 부운듯한 것. Necrotizing pancreatitis는 조영증강이 안되는 부위가 있는 것

처음에는 interstitial edematous pancreatitis처럼 보이지만 일주일이 지나면 necrotizing pancreatitis임.

acute peripancreatic fluid collection 은 나중에 pseudocyst가 된다.

fluid와 necrosis가 같이 있는 것으로 나중에 walled off necrosis가 된다. 

acute necrotic collection이 4주가 지나면 walled off necrosis가 된다. walled off necrosis는 pseudocyst와는 다른데

한마디로 말해서 건더기가 있다는 것이다.

따라서 drainage를 해야 하는데 제법 굵은 caliber의 catheter를 사용해야 한다.  반대로 pseudocyst는 맹물에 가까우므로 저절로 absorption되는 경우가 흔하다(ductal communication에 따라 management가 달라진다)

결론적으로

acute fluid collection에는 peripancreatic collection과 necrotic collection이 있는데 peripancreatic fluid collection은 4주후에 pseudocyst가 되고 necrotic collection은 4주후에 walled off necrosis가 된다는 이야기임.

pediatric osteosarcoma, Ewing's sarcoma, Rhabdomyosarcoma

osteosarcoma환자에서 tumor가 neurovascular bundle을 encase하고 있으면 limb salvage보다는 amputation을 해야 함. 이 환자는 abutting만 하고 있고 encasement가 없어 limb salvage를 했음.

OSA는 skip metastasis가 있으므로 반드시 두개의 joint를 포함해서 MRI를 찍어야 함

limb sparing과 amputation간에 local recurrence는 차이가 없다.

PNET(피넷이라고 읽더라) 가 Ewing's sarcoma family에 입적하였음.t(11;22)도 같고 chemoTx도 같다고 함.

Askin tumor는 rib destruction이 중요한데 그 이유는 비슷하게 생긴 pleural pulmonary blastoma는 rib destruction이 없기 때문임.

엠보싱(embryonal에는 botryoid와 spindle cell type이 있음)

특히 alveolar type extremity rhabdomyosarcoma에서 PET/CT가 유용함.
